The answer, in most cases, is no. One of the defining advantages of the All-on-4 approach is that a full-arch provisional restoration can be attached to the implants on the same day as surgery. This is what dental professionals mean when they refer to "immediate function," and it changes the experience of tooth replacement significantly.

Traditional implant treatment often required patients to wait several months between implant placement and receiving a restoration, leaving them to manage with a removable temporary option in the meantime. All-on-4 was designed with a different goal in mind. By using four strategically placed implants, including two that are tilted at an angle in the back of the arch, the system achieves enough initial stability to support a fixed provisional bridge right away. You leave the appointment with teeth already in place.

Why Immediate Loading Is Possible


The engineering behind All-on-4 makes immediate loading possible where conventional implant placement often cannot support it. The two posterior implants in the All-on-4 design are angled up to 45 degrees, which allows them to engage a longer section of available bone and avoid areas where bone density is lower. This positioning, combined with the four-implant distribution across the arch, generates what is called primary stability, meaning the implants resist movement well enough at placement to bear the controlled load of a provisional restoration.

The Provisional Restoration: What It Is and What It Is Not


The teeth you receive on the day of surgery are a provisional restoration, not your final set of teeth. This distinction matters. The provisional bridge is designed to look natural and function well enough for daily life during the months of healing ahead, but it is made from materials that can absorb and distribute bite forces in a way that protects the healing implants beneath.

During osseointegration, the process by which the implants fuse with the surrounding jawbone, protecting the implants from excessive force is essential. Your provisional restoration is built with this in mind. It will feel stable and look presentable, but our team will give you specific guidance on what to eat and what to avoid during the healing period. Soft foods, careful chewing habits, and avoiding hard or sticky items are all part of protecting your investment while the bone integrates around each implant.

What the Healing Period Looks Like


Osseointegration typically takes three to six months, depending on the individual. During this time, you will have follow-up visits at Timothy H. Kindt, DDS to monitor healing, check bite alignment, and make any adjustments to the provisional. Swelling and some soreness in the first week or two are normal, and most patients find the recovery more manageable than they expected.

Once healing is confirmed and the implants have fully integrated, you return to have your final restoration placed. This is the prosthesis you will wear long-term, and it is fabricated from more durable materials designed to handle the full range of daily biting and chewing. The transition from provisional to final is typically straightforward, and the result is a restoration that looks and functions like natural teeth.

Daily Life During Healing


Living with the provisional restoration is manageable for most patients. You can speak normally, smile confidently, and eat a wide range of foods, with some exceptions.

Your care team will provide a detailed list of what to avoid, but in general, the first few months call for:
•  Soft foods such as eggs, fish, cooked vegetables, and pasta
•  Cutting food into small pieces rather than biting directly into hard items
•  Avoiding hard bread, raw vegetables, nuts, and crunchy snacks
•  Skipping sticky or chewy foods that could place uneven stress on the provisional
•  Maintaining a thorough but gentle oral hygiene routine as directed

These guidelines are not permanent. Once your final restoration is in place and fully settled, most patients return to eating without restriction.

Who Benefits Most from Same-Day Teeth?


Immediate function is especially meaningful for patients who have been living with significant tooth loss or failing dentition for some time. Many arrive at their All-on-4 consultation having already given up on certain foods, avoided smiling in photos, or felt self-conscious about the way their teeth look. Walking out of the procedure the same day with a fixed set of teeth, however provisional, addresses those concerns right away rather than asking patients to endure a longer gap in function and appearance.

The psychological benefit of leaving with teeth in place should not be underestimated. It helps patients adjust to the new structure sooner, speak with less hesitation, and feel more like themselves throughout the healing process.
